Autonomous home care teams - more humanity for an attractive field of work

Demographic change affects nursing care in two ways: As the population ages, the demand for professional nursing care increases. At the same time, the labour force potential is decreasing because the baby boomers are increasingly leaving the workforce. Nursing care provided by autonomous home care teams is intended to improve the state of health of people in need of nursing care as well as the co-operation of all those involved in the nursing process. The focus is on people and their holistic care situation. The self-determination of people in need of care and nursing staff is to be strengthened and the nursing field of work made more attractive.

In the three-year model project, the nursing service Mook we gern gGmbH will develop new approaches in home care and test them in a pilot operation in Heide. The special feature of the model is that small autonomous home care teams organise themselves and their tasks themselves. A support network consisting of family, neighbours, acquaintances and voluntary helpers is being developed, and new procedural structures are also being created through this cooperation.

The goal of all those involved: the greatest possible autonomy for those in need of care within their own four walls. The nursing teams are testing a modified work organisation and thus a new organisational model for home care.

Team building measures support and accompany the establishment and development of the teams. The concept and the experience gained is being passed on to interested parties in Schleswig-Holstein.
 
DSN has supported the application in three funding programmes and is now involved in team building and development as well as project management. In addition, DSN is preparing a process manual to support the transfer of the model into regular care.
